Receiving Referrals

Following the receipt of your referral, we will review the information. Once we are satisfied that we can provide appropriate and safe care, we will contact you to discuss your requirements in more depth and assess what bespoke services we can provide to you.

If you would like to proceed, we will visit your home to undertake an assessment of your specific care needs and any risks posed. Thereafter, we will draft and cost your care package and present this to you. We are more than happy to speak to relevant and appropriate relatives about the care package and how we can best support you.

If you are happy with our care package, we will finalise the care plan with you, in conjunction with your family and / or relevant healthcare professionals (e.g. nurse, GP, etc).

We will identify the most suitable Carers to support you and arrange for them to visit you. We will always match the Carer with your interests and personality. If you are happy with your Carer, we will deliver any additional specialised training in order to provide you with the bespoke care and support you need. If you are unhappy with the Carer we have chosen, we will send an alternative person for you to meet.

Following the commencement of your care package, our Care Manager will visit you on a regular basis. This is to ensure that you are satisfied with the care we are providing and to make changes or improvements if needed.
